Riders under 16 yrs of age: I accept full responsibility for my child, and I accept my child rides at their own risk.
Riders aged 16 yrs and over: I confirm that I agree that I ride entirely at my own risk.
Data Protection Act 1998: Statement: I understand that the information I have given will be held in accordance with the data protection act 1998 but may also be made available to insurers and other concerned parties in the event of any injury or accident.
I understand that I must obey the instructions of the instructor and comply with the health and safety requirements of the establishment.
Riding is a risk sport and horses/ponies can be unpredictable, you may fall from the horse or receive injury. We advise you to ensure you have appropriate personal insurance cover. All riders must wear a hat conforming to current safety standards & suitable safe footwear & clothing, we reserve the right to refuse you to get on if you are not suitably & safely dressed.
I confirm that to the best of my knowledge all of the entered details are correct. A parent or guardian of riders under the age of 16 must sign this form.

Booking online is not currently available. In order to continue to deliver the safe targeted lessons & similar ability groups, here at Monach Farm Riding Stables, we have decided that our staff & instructors will continue to organise the lessons according to availability and suitability.
Lessons are currently being booked by our staff on a week by week basis & we will offer you lessons as soon as we have a safe suitable place for you.
We are not prepared to compromise on the safety & welfare of our team both 4 & 2 legged.
We are currently experiencing a very high demand for lessons & cannot guarantee regular spaces especially at weekends & after school.

Requests are taken up to 4 weeks in advance & confirmation sent on the Monday of the week requested. A request for a lesson does not guarantee we can book you in, but we will do our best not to leave you waiting too long.
